How To Register
Complete the registration form accessible below then email the form to SHeidel@ftba.com.

Space in the course can be reserved by phone or by fax, but in order to confirm a space, the completed registration form should be received at least ten working days prior to the course or your space will be released.

Cancellations:
If cancellation notice is received, the registration fees will be refunded, minus a $75.00 per person service charge. All cancellations must be received in writing.
No-shows will be responsible for the entire fee.
